Q. How can a care manager help me?
A. Elder Care By Design knows the local community and its resources. We help guide you through the fragmented long term care system, help you avoid costly mistakes and ensure your needs are met with appropriate services and products that support your goals. We act as an advocate for clients in difficult situations and help alert you to possible exploitation and scams. Care managers look at the "big picture" and can anticipate potential concerns or potential strengths that can help make a difference in the life of the older adult.
Q. When should I call a care manager?
A. The best time to call is before a crisis. Pre-planning can save money, time and worry about what to do when "a crisis happens." By hiring our care manager to assist you, a plan will be in place to manage the unexpected. If you observe any red flags, you should contact a care manager immediately.
Q. What is an Assessment?
A. Our assessments are comprehensive and individualized. Conducting the assessment in the older adult's home environment gives us an accurate understanding of the client's situation. Areas covered include: medical, nutritional and functional status; cognitive, psychological and social functioning; legal and financial status; safety and environmental needs. The assessment helps us develop a comprehensive plan to address all of the client's needs.
Q. What is the difference between home care agencies and professional care management?
A. Home care assessments are bound by law to stay within specific guidelines pertaining to their individual services. A professional geriatric care manager's assessment encompasses the total picture. We are in the unique position to assess, coordinate and assist clients with all aspects of care. Care managers work with home health agencies and other in-home service providers. The care manager acts as an "air traffic controller" by directing the flow and ensuring that everyone is working for the benefit of the older adult.

A. Costly mistakes can be avoided with needs accurately assessed and appropriate level of care in place. Locating and coordinating services can be a very time consuming and a frustrating experience for families. A. After competition of the comprehensive assessment, development of the care plan and implementation of the care plan; ongoing services and care checks are scheduled at intervals that work for the individual in order to keep things running as smoothly as possible. Change is inevitable and at this stage, we ask the right questions and provide the right amount of information and advocacy. By maintaining a relationship with the older adult, we are able to increase the trust level and this allows the care manager to better meet the needs of the individual.
