The individualized care plan is developed based on the comprehensive assessment and using the utmost clinical judgment and sensitivity.  This is a complex process that the care manager is specifically trained to do.  The plan maps out the next steps with the goal to preserve and protect the independence and dignity of the older adult by providing them with the most personalized client centered services to maintain the best quality of life possible.

Fee:  $675 per individual  $875 for couple
Includes: comprehensive assessment, care plan, referrals and arrangement of services


ONGOING CARE MANAGEMENT SERVICES

Despite the routine of daily life, changes are inevitable.  We work hard at anticipating changes and adapting the plan accordingly.  Ongoing services are scheduled at intervals to keep things running smoothly.  By continuously monitoring the care plan, we are able to review the client's status, services needs and evaluate the progress toward the goals identified in the care plan.

Fee:  $85 per hour
Billed in 6 minute increments
*Minimum 1 hour per month
